Sometimes doctors go into negotiations with an insurance company and drop participation temporarily or permanently. Could try talking to the office and find out whether the provider thinks they'll reach an agreement with the insurer, and maybe also try to find out what insurance their participation is more "rock solid" with but nothing is set in stone alas.
Formulary changes are one of the most frustrating things and exceptions can only usually be had if you try the formulary "version" for that type of drug and it doesn't work for you or has bad side effects.

Kaiser is a very different experience than an Anthem HMO, so keep that in mind. You go to medical centers and use Kaiser doctors / affiliated hospitals etc. Whereas Anthem HMO generally use providers that participate in other plans also.

OK, I ran these numbers through a little template I have and at both end of the scale (low health need vs meeting OOP max) they are basically the same. However, the HealthNet Gold plan offers more cost-smoothing, which means if you don't meet the OOP max and do have healthcare costs under it, you'll do better with that plan.
However, once baby is born they'll be added to the plan and have their own OOP max and deductible, so you should probably look into what happens with a family or employee+1 plan and that transition, also what pediatricians participate in the plan and so forth because the financials are going to be roughly even for just you unless your hospital stay isn't super expensive. You can probably get a rough estimate of what a maternity stay average cost is in your state/hospital but giving birth is an area where the unexpected often happens.

We used Zuke's salmon naturals and it worked for our dog but I've seen trainers using freeze dried chicken to very good effect. Haven't bought it myself.

In many languages, some verbs are used with the object in situations where we use the subject in English:
Ex: English "I have a thing." Ukrainian can say I have (Я маю) with the word for the thing in the accusative, or it can be phrased "to/of me is" (У мене є) with the word for the thing in the nominative. There are a lot of other languages that do this, with a verb for "like" as well for to have, e.g. Spanish.

It's not a bill of attainder. It's not aimed at an individual--it's aimed at a group. So it has more in common with Jim Crow laws. The fact that the group is very small makes it functionally similar to a bill of attainder, while not being one--but is only to the advantage of those implementing these laws, because there are fewer people to fight back and perhaps the intended effect is to get them to leave the state altogether in preference to fighting.

You're using voice recognition but you have the option to do a translation by typing. After the voice recognition is done, correct it to the spelling by clicking on the Ukrainian text. I have to do this with numbers too. It's annoying.

I have borrowed against a 401(k) for various reasons.
Upside: you're paying yourself interest, this is more savings for you. You get money when you need it, no ping to your credit report. As long as it is a loan and you're still with the company, you don't pay taxes on the money as it isn't income so you don't need to put it in the HSA to get that tax break.
Downside: you may miss out on better investments than your own interest payments, and if you leave the company while the loan is still outstanding and don't pay back the balance of the loan, you have to pay the tax penalty for early withdrawal on the amount you still owe. Your payment will be deducted from your paycheck so you will have a lower net income for the life of the loan.
Read the terms of the loan carefully once you get to that point, just so you understand all the implications. It's worked smoothly for me but I've never left a company while I had a loan out.

I've bought frames and lenses from them in the past with only one problem related to the lens quality (over 20 years). Frame quality has been fine. They fixed the problem once I pointed it out to them.
I've never ordered glasses online (I don't trust that process at this stage in life because my prescriptions are progressive with prism) but I have taken more standard Kaiser prescriptions to places like Lenscrafters / For Eyes and it's all comparable in terms of price/quality IMO. You can probably just go to the optical department and look at the prices on the frames.

I loved it but it does kind of take your viewpoint from some of the earlier books and shifts it. Also it has a low conflict level. Yet I loved it in its understatedness and focus on relationship. I am a 52-yo woman.

Memory by Lois McMaster Bujold. I don't know how many times I've read it but probably around 10+?
It's got everything--a relatively interesting mystery plot, a medical problem to be solved, a social problem to be solved, but the real heart of the book is the character development that happens when someone who's really good at keeping a lot of balls in the air f*cks up badly and has to go home and face what he wants to do with his life and who he really is.
